When you submit the M30 Performance intake form, we collect your name, email address, and other information that you provide. Please do not submit medical records, diagnoses, treatment details, passwords, financial information, or other sensitive personal information.

We use intake information to respond to your request, communicate about services, and understand demand for our programs. Submissions may be stored by M30 Performance and may generate an email to our team.

The form relies on Google, Cloudflare, and other service providers for hosting, security, communication, and data processing. These providers may process information as needed to provide their services. The site may also load content from third-party providers.

Retention and requests

We retain submissions only as long as reasonably needed for follow-up, operations, recordkeeping, or legal obligations. To request access to, correction of, or deletion of an intake submission, email fitness@m30performance.com.

Security

We use access controls and automated abuse checks, but no internet transmission or storage system can be guaranteed completely secure.
